Unhealthy meaning your character is sick, injured, or just in bad shape overall. What games make that a big deal?
Project Zomboid is brutal. Even catching a cold can get you killed if you can’t treat it properly.

Burnt toast? You die.

Zomboid is harsh. Scratches have a 7% chance to infect you. Lacerations? 25%. Basically, you can’t afford to take hits.
@Levi
Thankfully, you can change it in sandbox mode so only bites infect you. But yeah, default settings? A scratch can end it all.

The sandbox settings are awesome. I have multiple saves for different difficulties depending on my mood.

Or just turn off zombies completely. Makes it dull, though.
@Harlem
Yeah, turning them off is fun for a ‘last person alive’ vibe, but it gets boring fast. You can make it harder by reducing loot availability, but without zombies, it lacks excitement.
@Levi
On base settings, Green Hell feels even less forgiving.

Wound infections just slow down healing, but they’re still annoying to deal with.
@Logan
True, but ignoring them can spiral into a real problem.

Gotta pop vitamins before chowing down on Sham sandwiches and toilet water.
@Firth
Sounds like my college diet.
@Firth
Save those Sham sandwiches for antibiotics. Or just burn meat into biscuits of doom.
@Firth
Don’t forget to hydrate after the inevitable dysentery. Lol.

Make sure to boil the water first. Always.

Or get a purifier for your helmet slot.

The old wellness system made things even tougher. Cooking meals felt rewarding, and sickness or death hit harder.
